All Saints Recovery LLC (“All Saints Recovery,” “we,” “us,” or “our”) is committed to protecting the privacy of your health information and the information you submit through our website. This Notice describes how medical and personal information about you may be used and disclosed, how you can get access to this information, and how you can request records or file a complaint. Please review it carefully. This combined format follows the current federal model framework for healthcare provider notices, and federal guidance also allows a combined notice approach for Part 2 programs that are also HIPAA covered entities.

Your Rights

You have rights regarding your health information. Depending on the circumstances and applicable law, these rights may include the right to request access to or a copy of your record, request an amendment or correction, ask for confidential communications, ask us to limit certain uses or disclosures, receive a copy of this notice, and file a complaint if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

If your records include substance use disorder treatment information, some of that information may be protected by additional federal confidentiality rules. In those cases, your consent may be required for many uses and disclosures unless a specific legal exception applies.

Our Uses and Disclosures

We may use and disclose your health information for treatment, payment, and healthcare operations. This may include coordinating your care, communicating with other providers involved in your treatment, obtaining payment, conducting quality improvement activities, maintaining records, and operating our program. We may also use or disclose information when required or permitted by law, including for certain public health and safety purposes, valid legal process, reporting duties, and other situations allowed by law.

Uses and disclosures not otherwise permitted generally require your written authorization. You may revoke an authorization in writing, subject to legal limits.

Our Responsibilities

We are required by law to maintain the privacy of protected health information, provide you with notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect. We may revise this Notice from time to time, and any updated version will be posted on our website and made available upon request.

Through our website, including our contact form and admissions form, we may collect information you choose to provide, including:

  • Date of birth
  • Insurance information
  • Any additional details you include in your message

We use this information to respond to inquiries, communicate with prospective or current clients, evaluate admissions needs, verify benefits or eligibility when appropriate, and support our operations.

Website Tools and Technical Information

Our website uses Google Analytics and reCAPTCHA. These tools may collect technical information such as IP address, browser type, device information, and website interaction data in order to help us operate, secure, and improve the site.

Website form submissions are sent by email and are not stored through the website itself. Even so, no electronic transmission method can be guaranteed to be completely secure. Please avoid sending highly sensitive or urgent information through general website forms unless specifically requested.

Contacting the Licensing or Disciplinary Authority

Depending on the nature of your concern, you may contact one or more outside authorities.

For complaints involving licensed behavioral health professionals

Complaints involving licensed behavioral health professionals in Texas may be directed to the Texas Behavioral Health Executive Council (BHEC). BHEC states that complaints must be submitted using its complaint form. The completed form may be sent by email to Enforcement@bhec.texas.gov or by mail to:

Filing a Federal Privacy Complaint

You may also file a health information privacy complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR) if you believe your rights under HIPAA or the federal confidentiality rules for substance use disorder records were violated. OCR accepts complaints online, by email, or in writing. HHS states that complaints may be submitted through the OCR Complaint Portal, by email to OCRComplaint@hhs.gov, or by mail to:
